How to apply

This foundation reports that it accepts applications.

Applicants must make a written request or be referred. The customized medical plans then must be reviewed by a third party and in some cases the grant recipients must be interviewed by a third party. Upon acceptance a grant letter is given to the recipient describing what is being provided.

Deadlines: 12 31 2024

Restrictions: Applicants must have limited income that would otherwise prevent them from being able to pay for wellness programs for themselves. Priority is given to parents of young children or children themselves.
