PDF submitted electronically that includes: summary and history of program, website address, IRS establishment letter, two most recent 990s and audits if applicable, reason for application, amount requested and a proposed budget and schedule of how funds will be used.

Deadlines: Applications accepted year-round

Restrictions: Mission-driven organizations and Christian values-based programs; programs with an existing budget of $500,000 to $3 million; programs must be based in and serve the United States population
