Application to be completed with the following information: 1 - Personal and contact information 2 - GPA and senior courses being taken 3 - Record of community and school activities 4 - Post-secondary plans 5 - Essay on career goals and impact of an influential educator 6 - Three academic recommendations from teachers 7 - Transcripts and ACT scores verified by school officials

Restrictions: Scholarship funds are distributed by checks made payable jointly to the student and their designated college or university to ensure proper application toward educational expenses.
