How to apply

This foundation reports that it accepts applications.

Applications must be submitted in writing and include: name of pet owner; street address; phone number; email address; name of regular veterinarian; pets condition/diagnosis; estimated cost of treatment; donation request; copy of prior years tax return for the pet owner; proof of public benefits or social security or disability income, if appliable; and signed consent form.
