Proposals may include the following: a.Grant applicant's mission statement. b.Grant applicant's goals and objectives. c.The population served by grant applicant. d.Names and backgrounds of the grant applicant's board of directors. e.Grant applicant's budget. f.Grant applicant's project, title and description. g.Grant applicant's project strategy. h.Grant applicant's plan to monitor itself, and evaluate the success of its own project. i.List of grant applicant's senior staff and their experience. j.Information addressing grant applicant's leadership, e.g., staff turnover. k.Description of grant applicant's projects that have been successful, and analysis of reasons for success. l.Statement of why grant applicant is unique and identification of other organizations that operate in the same area. m.Other sources of funding for proposed project. n.Grant applicant's financial statement.
