WRITTEN GRANT REQUEST MUST CONTAIN THE ORGANIZATION'S NAME & ADDRESS, ITS PURPOSE, THE GRANT AMOUNT REQUESTED, AND DESCRIPTION OF WHAT THE GRANT FUNDS WILL BE USED FOR.

Restrictions: ALL GRANT RECIPIENTS WILL BE REQUIRED TO SIGN A GRANT LETTER AGREEMENT AND PROVIDE A COPY OF AN IRS DETERMINATION LETTER.
