The following needs to be sent in with application: Legal name and address of organization, copy of IRS Tax-Exempt ruling letter, copy of most recent state registration description of organization and a charitable mission specific description of how funds will be used, and amount requested and date request

Restrictions: Awards are limited to activities, projects, etc... that enrich youth
