In writing, in the form of a letter, with a statement of the need, and the amount requested.

Deadlines: Prior to the annual meeting each year.

Restrictions: Charitable, education and scientific purposes within the meaning of Sec. 501 (c) (3) of the Internal Revenue Code of 1986.
