Completed application Cover letter with a brief description of organization and request Audited financial statement opinion of Certified Public Accountant for most recent year Letter of Determination from the IRS verifying not-for-profit-tax-exempt status Current listing of Board of Directors and Officers

Deadlines: Applications are accepted on a rolling basis.

Restrictions: The MAXIMUS Foundation provides cash grants to nonprofit organizations/charities that help disadvantaged populations and underserved communities through youth and child development, community development, and healthcare. Applications are only offered to organizations nominated by eligible employees.
