Objectives: Specific program description; Type of person reached (age, geographic location, social and economic status), number of persons to be reached. Priority given to resident of Broward County and Florida. 3. How will funds be used.4. Line item operating budget and audited statements and tax return.5. IRS Determination Letter of 501(c)(3) status and verification that it has not been revoked.6. Funding sources with dollar amounts or received.7. Names of governing board members.

Deadlines: Accepted anytime provided sufficient time to review.

Restrictions: None, other than qualification for charitable purposes.
