How to apply

This foundation reports that it accepts applications.

Application form supplied upon request and must be completed accompanied by transcripts of all high school and other academic credits, at least three references from which recommendations are to be forward under separate cover, and must include financial and non-financial information.

Deadlines: April 1 of each year

Restrictions: Applicant must be a graduate or potential graduate of Community Unit District #1 High School, Carlyle, Illinois, and planning to attend four years of college.
