How to apply

This foundation reports that it accepts applications.

In addition to applicants name, date of birth, grade school, address, and phone number, the application requests the name of the applicant's guardian and reason for application. If applicable, the estimate of any procedure to be performed must accompany the application. In addition, the guardian's most recent tax return must be included with the application. Call for applications.

Restrictions: For health, welfare, & feeding of needy children under the age of 18 years.
