Contact Choate, Hall & Stewart for application. Application must include a brief history of the organization. Applicant must must supply a description of their project, the need for the project and describe their capability to complete the project. Questions to consider: What will be accomplished? Does it have a reasonable assurance of success? How does it seek to "break new ground"? If other funds are required, will the project be started before the balance is in hand? What criteria will be used to measure success? Applicant should also attach: 1) List of Board of Directors; 2) List of the five highest paid staff members; including salaries; 3) List of sources providing general operating funding; 4) List of sources providing funding for this particular project; 5) Detailed project budget; 6) Most recent financial statement; 7) Resume of person overseeing project; 8) Applicant's IRS exemption letter.
