Three-page written form including information on family, statement of preparation, employment while in school, requirement to provide transcript, and agreement to advise of academic progress at end of each semester.

Deadlines: Applications must be submitted no later than March 1st for the next academic year.

Restrictions: Recipients must be Maine residents pursuing a degree in nursing from a Maine institution of higher education. Scholarships are made in minimums of $1,000 and are renewable for up to four years.
